Care Coordinators provide non-clinical services in order to improve the medical compliance and health outcomes of our Medicaid members. This position identifies barriers to medical compliance, such as lack of transportation, illiteracy, or other social determinants that impact a member’s health, and ensures services are delivered and continuity of care is maintained. The position analyzes the home and community environment and makes autonomous decisions regarding appropriate care plans and goals, using a thorough knowledge of available community resources. These services are provided in the home, hospital, health care setting, via phone, and/or in person. Local, daytime travel is required through a reliable means of transportation insured in accordance with Company policy.

We perform many different types of Care Coordination, for example:
•Maternity Care Coordination works to reduce maternal and infant morbidity and improve birth outcomes.
•Family Planning Care Coordination works to reduce unintended pregnancies, promote pre-pregnancy health, and to improve the well-being of Alabama’s children and families.
•Pediatric Care Coordination for Children with Medical Complexity works to provide care and the linking of services for children with medical complexity.

Maternity Care Coordination requires a licensed ADN with 1 year experience in Care Coordination with low-income populations, an LPN with 2 years of clinical experience and 1 year experience accessing resources and coordinating care with low-income populations, a BSN, or a BSW. Preferred = LMSW; Certified Case Manager (CCM).

General Care Coordination and Family Planning Care Coordination require a licensed BSN or BSW. Preferred = Experience in case management, human services, public health, or experience with the underinsured population; LMSW; CCM.

Pediatric CMC Care Coordination requires a licensed LMSW with experience working in a pediatric environment. Preferred = LICSW; CCM; Experience working in a setting such as acute hospital, intensive care, children’s rehabilitation, children’s specialty clinic, children’s mental health, or pediatric clinic.

All require valid driver’s license in good standing, willingness to submit to vaccine testing and screening, and may require significant face-to-face member contact, with duties performed away from the principal place of business.
