The Care Coordinator coordinates services for Medicaid recipients in order to improve the management of chronic diseases. This position is responsible for education, counseling, follow-up, and resolving transportation issues. These services are provided via phone, in person, and/or in the home via local daytime travel through a reliable means of transportation insured in accordance with Company policy.

Requirements REQUIRED:
• Bachelor’s Degree in Social Work or Nursing, or equivalent experience
• Current RN or SW License in good standing with the appropriate State of Alabama Board
• Valid driver’s license in good standing
• Excellent interview and telephone skills, as well as the ability to deal with recipients in a caring and helpful (assertive and tactful) manner
• Working knowledge of health related service delivery systems
• Excellent communication and relationship skills
• Ability to analyze varied environmental factors in relation to members’ well being
• Ability to locate, augment and develop resources, including information on services offered by other agencies
• Organization and time management skills
• Ability to be flexible, adaptable, and to work effectively in a variety of settings
• Basic computer skills
PREFERRED:
• Master’s Degree in Social Work
• Certified Case Manager (CCM)
• Less than 1 years’ experience in case management, human services, public health, or experience with the under insured population
• Ability to utilize Microsoft Word and Excel
