SUMMARY: The Licensed Practical Nurse (LPN) provides nursing care and teaching to patients and families. He/She also functions as an assistant to the physician and registered nurse, and is supervised by the

• Graduate of an approved program of practical/vocational nursing
• Licensed as a licensed graduate practical nurse in the State of Oklahoma
• Minimum of one year experience as a nurse
• Effective communication skills, both oral and written
• Integrity to maintain confidentiality of information
• Knowledge of nursing theory and practice
• Works independently but performs as a member of the team
• Must have a car with required insurance coverage and a valid state driver’s license or reliable transportation

• Participates in the planning and coordination of total patient care in conjunction with the RN and the physician's plan of treatment
• Follows the nursing care plan for each assigned patient
• Observes the patient; evaluates the care of the patient; applies his/her knowledge of nursing skills to the patients to whom he/she is assigned
• Accurately reports and records the patient's condition and care, including signs and symptoms which may be indicative of change
• Prepares clinical/progress notes on the day of the visit and incorporates it in the clinical record per agency guidelines
• Assists the patient with activities of daily living while teaching appropriate self-care techniques.
• Provides and maintains a safe environment for the patient
• Provides optimum physical and emotional environment to provide support to terminally patients and their families
• Participates in interdisciplinary team meetings, in-service training, and other programs and meetings as needed or required
• Perform treatments as assigned per physician's plan of care
• Demonstrates competency/experience signed by an RN
• Maintains privacy and adheres to all HIPAA policies
• Complies with OSHA standards
• Participates in on call nursing rotation as assigned
• Perform all other duties as assigned

• Able to communicate verbally and in writing to the extent required by the position
• Able to physically perform the duties required by the position
• Able to travel to patients' residences
• Requires full range of body motion including handling and lifting patients, manual and finger dexterity and eye-hand coordination
• Requires standing and walking for extensive periods of time
• Requires the ability to lift and carry 50 pounds
• Requires corrected vision and hearing to normal range to record, prepare and communicate appropriate reports
• Requires working under stressful conditions and working irregular hours
• Requires some exposure to communicable diseases or body fluids
• Physical demands are in excess of those for light duty work

• Reports to Clinical Supervisor, Case Manager, and/or other Administrator’s as assigned
