Description JOB SUMMARY:
Acts as contact person with physicians, hospital discharge personnel, outside agencies, and families to complete admission process. Coordinates team members in arranging admissions.

ESSENTIAL JOB RESPONSIBILITIES:
Include the following. Other duties may be assigned.
• Receives all incoming referrals. Requests documentation to support hospice referral, as well as insurance/payor source information and documents regarding health care directives. Logs the referral and assigns a number to the patient. Makes appropriate copies and distributes to designated personnel.
• Requests and receives written referrals and medications from physicians.
• Enters appropriate information in computer program to begin patient’s medical record.
• Contacts the referring physician for certification and requests the patients records if needed. Faxes the certification to out-of-town physicians. Requests the physicians UPIN and ME#'s if not included on our physician listing.
• Informs the Hospital Liaison of all new hospital referrals from local hospitals; keeps liaison updated onall status changes on referral patients in the hospital.
• Assigns referral visit to an admission nurse and a social worker, or financial counselor if necessary, after scheduling the appointment with the patient or caregiver.
• Monitors bed availability at residential houses and keeps the PFCC informed of any requests for placement.
• Arranges for DME deliveries with the warehouse and the patient or caregiver as needed.
• Contacts Hospices in other counties when a patient living outside Marion County is referred.
• Completes Hospice consults, admission informed consent, SNF contract when appropriate, hospice house room and board contracts in all locations of referred patients served by Hospice of Marion County. This includes any location of patient or necessary family members i.e. private home, physician office, family member work place, SNF, ALF, hospital or clinic as requested.
• Identifies complicated needs of potential Hospice of Marion County clients during consult to assure critical DME and supplies are on hand at time of admission.
• Works rotating weekends and holidays as department needs indicate to assure available staff for admissions needs.
• Cross trained to provide consultations, consents and contracts as needed.
• Maintains accurate records of all referrals, admissions, and non-admissions.
• Complies with all Federal & State Hospice requirements and Hospice of Marion County’s Policies and procedures.
Requirements • High School Diploma or GED required.
• Three (3) years experience in medical setting required; hospice experience preferred.
• Must have a Valid Florida drivers License, maintain current auto insurance coverage and have access to a reliable automobile.
• Ability to explain hospice informed consent to referrals and inquiries in layman terms.
• Ability to work with all psychosocial groups in a professional manner.
• Ability to work unsupervised in the field, exercising discretion and independent judgment.
• Ability to read, analyze and interpret general business periodicals, professional journals, technical procedures and governmental regulations, as well as documents such as safety rules, operating and maintenance instructions, and procedure manuals.
• Ability to write routine reports and correspondence.
• Ability to speak effectively before groups of customers, patients/families or employees.
• Knowledge of medical terminology and ability to demonstrate essential usage.
• Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
• Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
• Ability to operate phone system, a facsimile, copy machines, computers and other common office machines, with working knowledge of MS Word, Outlook, Excel and other software appropriate to positions.
