Description JOB SUMMARY:
Assists patients and their families with issues that interfere with obtaining maximum benefits from medical care. Provides community resource coordination by performing the following duties.

ESSENTIAL JOB RESPONSIBILITIES:
Include the following. Other duties may be assigned.
• Maintains effective communication with Hospice Admissions Department, Volunteer Department, and Bereavement with regard to referrals.
• Interviews patient/family unit/significant others to assess patient's situation and potential psychosocial and discharge needs.
• Develops and implements a discharge plan for the patient in conjunction with the patient/family significant others, and hospital social workers/discharge planners.
• Carries out responsibilities consistent with the standards and guidelines of Hospice of Marion County, the hospitals, nursing facilities and the profession.
• Maintains ongoing contact with the social workers/discharge planners in local hospitals and nursing homes to facilitate the referral process and maintain continuity of care.
• Provides Social Services consistent with knowledge, training, experience to the patient in a safe environment.
• Identifies Social Services needs of the patient/family unit and develops goals to meet those needs.
• Completes a financial summary on patient/family units as relevant for placement needs or referrals, assists the patient/family as needed, in finding financial support.
• Participates in the IDT meetings as an active member and contributes toward problem solving/goal setting.
• Works in close collaboration with health care personnel in patient evaluation and treatment to further their understanding of significant social and emotional factors underlying patient's health problem
• Helps patient and family through individual or group conferences to understand, accept, and follow medical recommendations.
• Provides service planned to restore patient to optimum social and health adjustment within patient's capacity.
• Utilizes community resources to assist patient to resume life in community or to learn to live within limits of disability.
• Possesses awareness and acceptance of various life styles without imposing personal values on the patient/family unit.
• Documents document interventions with patient/family accurately and in a timely manner.
• Maintains current knowledge of Medicare/Medicaid benefits in relation to admissions procedures and qualifications for skilled nursing facilities (SNF's), Co-op, private insurance, supportive care, continuous care and in-patient requirements.
Requirements • Bachelor of Social Work from a school accredited by the American Council on Social Work Education or related field. A Bachelor degree in a related field with additional years of social work experience may be substituted for BSW degree.
• Minimum of 2 years experience in social work required.
• Follows all safety precautions when lifting/moving heavy items and properly utilizes necessary lifting/moving equipment.
• Must have a valid Florida Driver's License, maintain current auto insurance coverage and have access to a reliable automobile.
• Ability to work unsupervised in the field, exercising discretion and independent judgment.
• Ability to read, analyze and interpret general business periodicals, professional journals, technical procedures and governmental regulations, as well as documents such as safety rules, operating and maintenance instructions, and procedure manuals.
• Ability to write routine reports and correspondence.
• Ability to speak effectively before groups of customers, patients/families or employees.
• Knowledge of medical terminology and ability to demonstrate essential usage.
• Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
• Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
• Ability to operate a facsimile, copy machines, computers and other common office machines, with working knowledge of MS Word, Outlook, Excel and other software appropriate to positions.
