Description JOB SUMMARY:
As a member of the Admissions Department and the Interdisciplinary Team, the Admission Nurse performs admission functions and is responsible for the initial nursing assessment by conducting a clinical and psychosocial evaluation and originating the plan of care. Coordinates care for all patients and provides nursing care, education and support to the patient, family and/or caregivers. Promotes professional integrity and the highest levels of customer service.

ESSENTIAL JOB RESPONSIBILITIES:
Include the following. Other duties may be assigned.
• Processes admissions on a 24 hour, 7 day a week basis as directed by patient and family needs.
• Works late evening or late night admissions at least once weekly.
• Provides professional, courteous nursing service to the patient consistent with knowledge, training experience and hospice standards.
• Provides the initial patient and family assessment, incorporating psycho-social, spiritual, cultural, physical and appropriate nursing interventions.
• Meets or exceeds weekly productivity standards for completing admissions.
• Demonstrates clinical skills regarding case management, such as IV skills, pain management titration, symptom management and patient/family relationships.
• Consults and collaborates with other professionals to promote the highest quality patient care in a timely manner.
• Implements and coordinates plan of care and begins case management of patient. Assesses the patient's physical and emotional condition at time of admission.
• Demonstrates understanding of Interdisciplinary Team concept, cooperating and working with all team members for effective patient care. Demonstrates positive team work behavior.
• Educates Patient/Family/Caregiver/Nursing Home Staff on administration of medication, symptom management and pain control as directed by physicians and Hospice of Marion County, Inc. standing orders and protocol when applicable.
• Maintains current knowledge of Medicare/Medicaid benefits, private insurance, supportive care, Crisis Care and in-patient requirements.
• Maintains confidentiality of patient/family information regarding disease, diagnosis, financials, or other information obtained during the course of employment.
• Adheres to professional standards of practice for nursing.
• Maintains an up to date knowledge and understanding of local, state and federal laws impacting upon the practice of nursing in the Hospice setting.
• Uses good body mechanics when lifting or transferring patients, utilizing equipment when necessary.
• Maintains Standard Precautions and uses necessary personal protective equipment, in order to maintain a safe environment in conjunction with OSHA regulations.
• Completes, submits, and maintains all required documentation in a timely manner according to policy, including but not limited to; Time and Travel Logs; Phone Logs and Admission consents.
• Collaborates with primary physician at time of admission to ensure optimal symptom control and to initiate the plan of care.
• Participates in the orientation/training of new staff through shadowing, communication of acceptable performance standards, promotion of Hospice philosophy and procedures.
• Responsible for ensuring complete and accurate information is provided to patients and caregivers about hospice care, The Federal Healthcare Programs Hospice Benefit, its criteria for eligibility and any limitations of coverage.
Requirements • Minimum of AA degree in nursing; Bachelors Degree preferred. Current Florida RN license in good standing.
• Minimum of (1) one year clinical nursing experience.
• Excellent assessment skills required.
• Current CPR Certification required.
• Valid Florida Driver’s License, current automotive insurance coverage and access to reliable transportation.
• Must be courteous, polite & willing to follow hospice process; to advocate for hospice services where applicable.
• Demonstrated ability to multi-task, handling caseload, stressful situations, and providing caring, compassionate customer service throughout.
•Ability to read, analyze and interpret documents such as safety rules, technical procedures, operating and maintenance instructions, government regulations and policy/procedure manuals.
• Ability to work with mathematical concepts such as probability and statistical inference. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations such as medication dosage.
• Ability to write routine reports and correspondence.
• Ability to speak effectively with groups of customers, patients/families or employees.
• Knowledge of medical terminology and ability to demonstrate essential usage.
• Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
• Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
• Ability to operate a facsimile, copy machines, computers and other common office machines, with working knowledge of software appropriate to positions.
